Basic Life Insurance
The College provides employees with life insurance equal to one times the employee's annual salary rounded to the next $1,000 up to a maximum amount of $200,000. and includes AD&D (accidental death & dismemberment) coverage.
Voluntary Term Life Insurance
The College offers optional term life insurance for employees, their spouses and dependent children. This coverage is paid by the employee through payroll deduction. See below for detailed information on premiums and information on applying for coverage.
Applying for Voluntary Life Insurance Coverage
To apply for additional life insurance outside of your initial date of eligibility, you will need to complete as Eveidnece of Insurability Form. The completed form is forwarded to Mutual of Omaha where it goes through the underwriting process. To begin this process, please complete an Eveidnece of Insurability Form by using one of the options below.
Evidence of Insurability Form (online submission)
To change your beneficiaries for life insurance, you will need to complete the Beneficiary Form and return to Human Resources.
Change Life Insurance Coverage
To end or reduce your optional life insurance coverage, you will need to complete the Change of Coverage Form and return to Human Resources.
