Research Project Forms

There are two different forms for Roanoke College's Institutional Review Board. If your research project involves human subjects, you must complete one of the forms AND you need to include all materials related to your project: study information sheet/ informed consent document, if needed; if conducting a survey or interview, the questions need to be submitted in Word or pdf form as well as a link, if being sent out electronically; and CITI certificates for all participants. Applications will not be considered until all relevant documents are submitted.

1-Exempt Registration Form

2-Expedited/Full Application Form
Complete this form if your research involves human subjects and does not meet the criteria of one of the federal Exemption Categories

Distribution Request Form

Anyone planning to send research related material (i.e. a survey, a survey link, an invitation to participate in a research project, etc.) to Roanoke College students, faculty, or staff must first obtain approval.  In addition to the human subjects protection review done by the IRB, requests to send material to members of the Roanoke College community will be reviewed by several administrative offices. You survey must have a Study Information Sheet as an introduction in order to be distributed to a campus group.

Approval for electronic distribution of material to a Roanoke College group is a 2 Step process:

Step 1: Complete the survey distribution request (You must first complete either the Exempt Registration Form or the Expedited/Full Application Form from the steps above).

Step 2: Email a copy of your material (PDF or Word) to IRB@roanoke.edu AND IR@roanoke.edu. Your survey must have a Study Information Sheet as an introduction in order to be distributed to a campus group.

 
Your application will not be processed until both steps are complete.
