Basic Life Insurance
The College provides employees with life insurance equal to one times the employee's annual salary rounded to the next $1,000 up to a maximum amount of $200,000. and includes AD&D (accidental death & dismemberment) coverage.

Voluntary Term Life Insurance
The College offers optional voluntary term life insurance for employees, their spouses and dependent children. This coverage is paid by the employee through payroll deduction. See below for detailed information on premiums and information on applying for coverage.
Applying for Voluntary Term Life Insurance Coverage
To apply for voluntary term life insurance outside of your initial eligibility, you will need to complete medical underwriting. To begin this process please compete Evidence of Insurability (EOI) Form (located below).
Evidence of Insurability Form (online submission)
Once the Evidence of Insurability (EOI) form have been received the insurance company, Mutual of Omaha will evaluate the application, health information, and other info provided and follow up should any additional information be needed.
Change Voluntary Term Life Insurance Coverage
To end or reduce or change your voluntary term life coverage, please contact Human Resources.
Beneficiary Changes
To change your beneficiaries for life insurance, you will need to complete the Beneficiary Change Form, located on FormFinder and return to Human Resources.
Disability Insurance
Short-Term Disability
All full-time and part-time regular staff will become eligible for short-term disability insurance after 6 months of service. Please see the short-term disability plan summary for the details of coverage.
Faculty short-term disability and medical leave is outlined in the faculty handbook.
Long-Term Disability
An employee is eligible for long-term disability insurance after one year of service. The disability plan is designed to protect you and your family from financial hardship caused by loss of income, and the premium is paid by the college. Please see the long-term disability benefit summary for the details of coverage.
