New Student Health Information

New incoming first-year students' health forms & required immunization information must be received by the college no later than June 1st for Fall Semester and January 8th for Spring Semester.  

Your Required New Student Health Forms

Student Health Portal        

  1. Login with your RC Username and Password
  2. Click the forms tab
  3. Complete Form 1. Tuberculosis Screening
  4. Complete Form 2. New Student Requirement Form 
  5. Click the document upload tab, to submit a copy of your immunization record and if indicated a PPD skin test results and/or QuantiFeron Gold-TB test or Chest X-ray results and insurance information.

Once you have submitted your health forms, they will be reviewed and you will receive a notification on your Roanoke College email that you have a secure message on your student health portal concerning any missing or incomplete information that needs to be submitted. This process can take up to 21 business days after submission. If you submitted your information and are still getting weekly notifications, please contact our office.
