Required Immunization and Prematriculation Guidelines
Health Record Documentation Instructions-Information for Students and Parents
All information is due by June 1st for fall semester and January 8th for spring semester.
All full-time students, including residents, adult and transfer students are required to complete the Medical and Immunization history and Tuberculosis Risk Assessment forms on the student health portal at https://roanoke.studenthealthportal.com and upload a current copy of your immunization history (see required immunization list below) using the “Document upload” tab on the portal.
All Immunization documentation must be obtained from your healthcare provider’s office or State Health Department.
Tuberculosis Risk Assessment Form (see portal)
If you answered “YES” to any of questions on the Tuberculosis Risk Assessment Form, a PPD skin test or
QuantiFeron Tb Gold (QFT-G) blood test is required. If either of these are positive additional testing will be required. All test results must be within 12 months of semester start date and all results uploaded to the portal.
The following immunizations are required based on Commonwealth of Virginia Law (Code in Virginia, Section 23-7.5) which requires that all full-time students submit an immunization history that has been provided by a health care provider, and recommendations from the American College Health Association (ACHA):
Required Immunizations:
- MMR (measles, mumps, rubella) 2 doses. On or after 1st birthday and at least 28 days apart
- Tdap booster must be within past 10 years
- Polio childhood series 1 dose must be on or after 4th birthday plus any boosters you have received
- Hepatitis B: (3 dose series) or titer results showing positive immunity
- Meningococcal Quadrivalent (ACWY) vaccine given on or after 16th birthday
- Varicella (chickenpox) a history of chickenpox disease, 2 doses of vaccine or positive varicella titer results
Recommended Immunizations
- Quadrivalent Human Papillomavirus (HPV) vaccine series
- Hepatitis A: 2 doses
- Meningococcal B series
- Influenza: All students are encouraged to receive annually during autumn to minimize disruption of routine activities during campus-wide epidemics. Health Services will hold flu shot clinics open to all students.
- Covid-19: Completed series- based on manufacturer specifications (Pfizer and Moderna- 2 doses; Johnson and Johnson- 1 dose)
Incomplete health information
After submitting your required information, it will be reviewed by our office. If there is, any missing or additional information required you would receive an email in your MyRoanoke account asking you log in to the student health portal to review a secure message. This will explain what is needed to complete your health information.

Additional information
Medical records and prescriptions:
Have a record of your prescriptions, including dosages and diagnosis/reason for treatment. If you have chronic/serious medical conditions, have a medical summary sent to Health Services and Counseling Services by your treating physician.
**If you are currently prescribed medication for ADD/ADHD, and will be requesting prescription refills while at school, a letter from your provider with documentation of diagnosis and medication dosage is required and must be dated within 6 months of semester start date. Refills are approved on a case-by-case basis.
Allergy injections
Health Services gives allergy shots at regularly scheduled times by appointment. A physician’s
detailed order is required. If initiating allergy injection therapy, we require the ordering physician give
the first injection. Students must wait 30 minutes after injections.
First aid supplies
You may want to bring a digital thermometer, over the counter cold and pain relief medications, Band-Aids, topical antibacterial cream and a hot/cold pack.
Health Insurance
Domestic students review medical insurance to ensure your policy provides adequate coverage while living at college for services such as emergency care, lab tests and x-rays. Carry your health insurance card and prescription drug card with you. Please upload a front and back copy of your health insurance card to the student health portal.
International students please review the letter you received from International Education Office regarding your insurance requirements.
